Please print out, complete, and bring both the Welcome Packet and the Health form with you to camp. Please read the information in both forms carefully.
If your child has a serious medical condition, allergy or disability, please contact us prior to camp.
If you are travelling to a camp from out of area and need help coordinating transportation (such as from an airport) please contact us as soon as possible at info@yeacamp.org
Please note that your child cannot attend camp without these signed forms!
